Transition is the process of planning and moving on from children’s to adult services. The time that you move on will vary and depends upon when you are ready but we will start to have discussions about this from the age of 14.

We understand that this time can be daunting. If you have a long-term condition that means you require ongoing Physiotherapy support, we will support you to move to adult services. We will work with the Adult Physiotherapy Team before you move on so that they have a really good understanding of your needs. We will meet with both you and your new Physiotherapist before you leave our service. We will also ensure that you, your family or carers feel confident in how to manage your condition and how to contact services should you need them. We will work with all health professionals involved in your care, social care, education services and anyone else who may be involved in your care after you move on so that your move is as coordinated as possible.

There are some young people who do not meet the criteria for ongoing adult physiotherapy services or we may agree that you are able to manage your own condition. In these circumstances you will be discharged from Children’s Physiotherapy and back to your GP. We will do our best to help you fully understand your health condition and how you and your family or carers can manage it before you move on. We will ensure you know how to access Physiotherapy in the future should you need it.
